A new Community Concepts initiative bringing Remote Patient Monitoring and Assistive Technology to rural communities across South Carolina, in support of the SCDHHS Connections to Care Initiative.
About the Initiative
Care Without Borders is designed to close persistent gaps in access to consistent, quality healthcare for residents of rural South Carolina. Through Remote Patient Monitoring and assistive technology, the program will connect individuals managing chronic conditions, and individuals with developmental disabilities, to the tools and support needed to stay healthier at home. The initiative reflects Community Concepts' continued commitment to expanding equitable, technology-enabled care across the communities we serve.
Remote Patient Monitoring
Connecting individuals managing chronic conditions to at-home monitoring tools and coordinated clinical follow-up.
Assistive Technology
Equipping individuals with developmental disabilities with technology that supports greater independence and safety.
Care Coordination
Interdisciplinary case reviews and community-based navigation to strengthen continuity of care across rural counties.
Digital Health Literacy
Training and support to help patients and families confidently use health technology in their day-to-day lives.
